Poor communication and action care plan not being followed

it feels as if the psychiatrist simply do not care or their workload is too much, resulting in sub par delivery of a care service which is devoid of any care. The issue runs deeper because the patients who suffer from mental illness may not have the confidence to vocalise the frustration they are having to endure at the hands of a service, which appears to be quite heartless.
When was the last time this centre was inspected? I doubt it has.
