Birmingham Community Healthcare NHS Foundation Trust has commenced work to review the impact of travel and transport in and around four of their key sites. These sites are:

• Birmingham Dental Hospital
• Moseley Hall Hospital
• West Heath Hospital
• West Midlands Rehabilitation Centre

As part of this review they are encouraging local people who live in the vicinity of these sites to share their views. Your views will help the Trust to develop a Travel and Transport Strategy.

• The survey will be available until the 16^th December 2016.
